    E. CSPMP database access                 1. A medical practitioner that chooses to use the CSPMP database shall request access from the CSPMP Director on forms provided on the Board’s website. Upon receipt of the request, the CSPMP Director or designee shall issue access credentials provided the medical practitioner is in compliance with the registration requirements of this section and has completed the Board’s CSPMP Online Training Program.                 2. A pharmacist that chooses to use the CSPMP database shall request access from the CSPMP Director on forms provided on the Board’s website. Upon receipt of the request, the CSPMP Director or designee shall issue access credentials provided the pharmacist has a current active pharmacist license and has completed the Board’s CSPMP Online Training Program. see full law
